Every vendor shows you a 45-second demo where the caller speaks perfect English and wants exactly one service. Real calls aren't like that. Here's what happens on a typical Tuesday at a dermatology clinic using our AI receptionist — transcript lightly edited for privacy, timing real.

That call worked because the intent was clear, the service existed in the catalog, and availability was already synced from Google Calendar. The AI didn't "understand medicine" — it matched "rash" to the consultation service and ran the same booking flow your web page uses.

What happens under the hood

Speech-to-text runs in real time (we use a mix of vendor STT with fallback). The LLM layer — GPT-4o and Gemini in our stack — sees your service list, staff roster, policies, and open slots. It's not improvising prices; it reads your configured catalog. When the caller confirms a slot, we write the booking the same way the web UI does, trigger SMS, and log a transcript in your dashboard.

Latency matters. Under 800ms turn-taking feels natural; over 1.5 seconds and callers talk over the AI. We optimize for the former on wired lines; mobile callers with weak signal are still harder.

Edge cases we handle well

Accents and mild background noise — modern STT is good enough for booking intents if you limit domain vocabulary. Hesitation ("um, maybe Tuesday?") — the model asks a narrowing question instead of failing. After-hours calls — the whole point; 38% of salon booking calls in our data happen outside 9–5. Language switching mid-call — supported on six languages on our AI plan; not magic, but scripted handoffs work.

Where it still breaks

Angry callers who refuse to talk to a machine — some want a human immediately; we offer press-0 escalation but not every clinic configures it. Multi-service marathon bookings ("facial, then nails, then massage") — possible but higher error rate; we recommend web booking for complex carts. Medical triage — the AI must not give clinical advice; clinics need tight service scopes. Novel requests outside your catalog ("do you treat iguanas?") — it should say no, gracefully. If your catalog is empty or stale, every call fails quietly.
